Effective June 1, 2008 the PPO Health Insurance Plan will have the following benefit changes:

1. A new Rx Prescription Drug plan provider, Atlantic Prescription Services (APS).   APS will send each member a new Rx drug card and welcome letter detailing their services.  This Rx drug card will replace your BeneScript Rx card effective June 1, 2008.  The Rx drug co-pays have increased to the following:

Generic - $5.00

Brand (Preferred) - $15.00

Brand (Non-Preferred) - $25.00

Mail order is 2 times Retail - (90 day supply)

An improved dental insurance benefit.  The dental insurance schedule that compares the present and new benefits is as follows:

Deductible - $25.00

Preventative - 100% / No Deductible

Basic Services - 80%

Major Services - 50%

Annual Maximum - $1,500.00

Orthodontia - 50% of Usual and Customary

Orthodontia Lifetime Max. - $2,000.00

An improved vision insurance benefit.  Vision insurance schedule that compares present and new benefits is as follows:

Eye Exam (1 per 12 mos.) - $50.00

Lenses (1 per 12 mos.)

Single - $75.00

Bifocal - $100.00

Trifocal - $125.00

Contacts - $150.00

Frames (1 per 24 mos.) - $75.00
